Email Address: This is a required field. If you have a Mines email address, this is preferred since it gives us some verification of who we are trying to help. Some sensitive information can not and will not be sent to non-Mines email addresses.
NOTE: If you are reporting problems with your Mines email account -- for example, a problem in which you are having difficulties getting or retrieving your email, please include another email address or other contact information so that we can contact you.
Name: Your name is not required, but it may help us resolve your problem.
Department: Your department affliation is not required, but it may help us resolve your problem or redirect your Support Request to the appropriate support staff.
Phone: Your phone number (on campus, home or cellphone) is not required, but some problems are more easily resolved via a phone call.
User Name: If your Support Request is related to an account problem, please include your campus or account User Name.
